The Bereavement Ministry assists mourning families in requested areas of need during times of death within a family. Duties include assisting in the preparation and completion of funeral arrangements, responding to specific needs of the family, participating in the eulogistic services when requested, and providing ongoing spiritual and emotional support.

At the bottom of this page is a form for the purpose of sharing information with the Mt. Level church family for both immediate family members (spouses, children, parents, grandparents and siblings) and extended family members (aunts, uncles, nieces, nephews, cousins).

The person completeing the Bereavement Ministry Form should be at least 21 years of age, and a designated member of the family responsible for submitting the form. The information should be printed or typed. 

Instructions for submission:

1. Submit only one form per deceased person. All other forms received for the same person will be discarded.
2. Forms should be submitted Monday through Friday, at least three (3) days prior to the date of the services. Forms submitted on Saturday and Sunday will not be reviewed until the following Monday. If the services are held on Monday, please submit the form no later than the Thursday prior to the service by 12 o'clock noon.

Please note: If you do not have computer access, you may call the church office for assistance. Please have the pertinent information available concerning the deceased family member and the service. If the Administrative Assistant is not available to take your call, leave a message stating the reason you are calling with your name and number where you can be reached.
